Jane Doe, a 32-year-old woman with a history of PCOS and dyspareunia, experiences a therapeutic journey that combines psilocybin-assisted therapy with traditional methods to address complex sexual and emotional challenges. With emotional challenges and the impact of her conservative Mormon background, the study explores her journey using psilocybin-assisted therapy, combined with internal family systems, to address the persistent pain, guilt, and shame impacting her intimate life. Notable challenges include the careful tapering of SSRIs, ethical and legal considerations around psychedelic use, and distinguishing between anxiety driven by psychiatric conditions versus social and religious influences. The case provides a compelling look at the integration of traditional and emerging therapies to resolve complex sexual and emotional health issues.

Explore the case of Edward and Colette, an aging couple who present to therapy with differences in their desire for sex. Throughout the course of therapy, complex factors emerge as possibly contributing to the state of their current (lack of) sexual relationship, including differences in desire, underlying questions about sexual orientation, hostile/avoidant relationship patterns, and factors related to each partner’s own individual backgrounds. But many questions remain unanswered. Challenges in the therapy include maneuvering conflicting goals for therapy, whether and how to address possible differing (but not named) sexual orientations as well as how to know when to terminate the therapy or treatment plan. Grieving losses around sexuality as one expects it to be and differentiation around finding one’s own sexual sense of self are also explored.
